The aim of this assessment is for students to develop an understanding of continuing care for
long-term and chronic conditions to meet the learning outcomes 1, 3 & 4.
CONDITIONS

TASK DETAILS / INSTRUCTIONS
Using one (1) of the four (4) case studies listed below, you need to address the following four (4)
points in your essay.
1. You, the registered nurse (RN), are meeting for a routine appointment with the patient
six (6) months after you last saw them. Utilising a research study, explain one (1) point of
education that the RN will focus on with the patient about their current long-term
condition and discuss rationale for why this education is important, linking to evidencebased
practice (600 words).
2. Discuss how the RN can demonstrate cultural safety when providing the education about
the long-term condition for the chosen individual and their family/whānau (200 words).
3. Utilising the Chronic Care Model and one other model of hauora chosen from the list,
give a brief explanation of the two models and compare and contrast the models (300
