Scenario
You are on clinical placement and working with a registered nurse (RN). You and the RN have
cared for Lisa1 since her admission three days ago with a diagnosis of left lower lobe
pneumonia. Lisa is due for discharge and has expressed concern about going home. She is
36 and has two children, ages two and five. Her partner often travels for work. Lisa works 25
hours a week preparing financial accounts for a local company. She does this work from home.
This clinical case study requires you to utilise the comprehensive health assessment
framework (supplied) to collect subjective and objective data about your patient and create a
care plan using the components of the Nursing Process (ADPIE). You will explain the
pathophysiology of pneumonia and its clinical manifestations, then apply this knowledge to
Lisa`s clinical assessment. Also, discuss why Lisa may be concerned about going home.
This assignment requires you to:
• Explain the pathophysiology of pneumonia and discuss the clinical manifestations.
• Discuss the physical assessments you would perform on Lisa, health history
questions you would ask, and diagnostic tests that may be required.
• Document a nursing care plan for Lisa using the ADPIE framework listed in order of
patient/person preference.
• Limit the nursing care plan to the first 24 hours of admission to the hospital
ward.
• Discuss Lisa’s concerns about going home and the implications of Lisa`s discharge for
herself and her family and provide recommendations for support.
